Alberta Health Services - Calgary Area Patient Safety Initiatives

The AHS - Calgary area has implemented many patient safety initiatives, including 92 of the 121 recommendations from the 2004 Robson Report.  Following are a few examples:

ICU Outreach – Code 66

Safer Healthcare Now!

The AHS - Calgary area participates in the national Safer Healthcare Now! collaboratives which currently operate six targeted interventions:

  Improved care for Acute Myocardial Infarction

  Prevention of Central Line-Associated Bloodstream Infection

  Prevention of adverse drug events through medication reconciliation

  Rapid Response Teams (ICU Outreach Code 66 in the AHS - Calgary area)

  Prevent surgical site infection

  Prevention of Ventilator-Associated Pneumonia

Human Factors

The AHS - Calgary area is one of the first in Canada to employ Human Factors consultants to review and redesign equipment and work areas for improved efficiency and safety.

Executive Walk-Rounds

AHS Calgary area senior leadership continue regular planned (and spontaneous) exchanges with front-line staff in order to learn of their experiences and perceptions as well as to share the AHS - Calgary area's commitment to patient safety Executive walk-rounds are routinely done now in all clinical portfolios.

The AHS - Calgary area promotes quality care and patient safety as core values and guiding principles.

Education and Training

The AHS - Calgary area has expanded training programs in investigation techniques for adverse events and close calls. 200 individuals have been trained in Health System Safety Analysis since November of 2005.

  Inpatient and outpatient pharmacy areas built at ACH

  Oncology Satellite Pharmacy at FMC renovated

  Inpatient Pharmacy renovated at FMC and new Outpatient Pharmacy built

  Satellite pharmacy for NICU at FMC built

A dedicated budget, resources, and infrastructure are allocated to patient safety in the AHS - Calgary area.
