Referral and Triage information and forms

Geriatric Assessment and Rehabilitation Program (GARP)

Referrals can be made by fax to 403-212-1230. For more information, please call 403-943-3453.

Hospital Consultation Services

Referrals are made by entering an “MD consult” order on the electronic health record (Sunrise Clinical Manager or SCM). The name of the Geriatrician on-call for each Calgary Region site can be determined by searching the Regional On-Call Application. To do this, choose the Area of “Internal Medicine” and Service of “Geriatrics.”
