To refer a patient to the Southern Alberta Clinic, please fax a completed referral form to us.

Southern Alberta Clinic Referral Form

Filling Out the Referral Form

When making a referral, please include the following information in order to avoid delay:

  • First name and last name
  • Provincial health care number
  • Sex
  • Date of birth
  • Positive HIV result

In addition, the following information will help us arrange the best possible care:

  • All relevant lab results, including recent CD4 and viral load results
  • Medication history
  • Relevant co-morbidities, especially active infections (e.g., tuberculosis)
  • Recent hospital admission records (within past year) including location and approximate dates
  • Language needs
  • Barriers to care (e.g., transportation issues)

All referrals to the Southern Alberta Clinic are considered routine and are treated equally. Currently we are able to accommodate new referrals within one to two weeks.

Please note that SAC does not perform HIV screening tests. All patients must have a documented positive HIV result before being referred. See a list of HIV Testing Sites in the Calgary area.
