Chronic Pain Consult Service

About the Chronic Pain Consult Service

• Offered at all 3 adult hospitals (FMC, PLC, RGH)
• Available Monday thru Friday
• Consult team consists of Nurse Practitioner, Clinical Nurse Specialist, Nurse Clinician, and Chronic Pain Physician
• Consult may also include an Addiction Specialist, Pain Psychologist, and Pharmacist
• The team assesses the patient and recommends treatment options for the attending physician to implement
• The team provides recommendations for the family physician upon discharge
• Educates patients regarding how to manage their chronic pain in the community after discharge from hospital
• Provides education to health care professionals

How to refer your patient to the Chronic Pain Consult Service

• Patients in the hospital with poorly controlled chronic pain can have their attending physician request a consultation
• Attending physicians can refer to the chronic pain consult service through departmental referrals in SCM
