Community Prevention of Childhood Obesity

Childhood obesity in Canada and many countries worldwide has reached epidemic proportions. Between 1981 and 1996, the prevalence of overweight children has tripled.

The Calgary Health Region formed a multi-sectoral steering committee in the fall of 2000, Community Prevention of Childhood Obesity, to address this issue.

Diverse Populations Chronic Disease Management Program

This is a community-based and culturally-sensitive chronic disease prevention and management program that is specifically designed for ethnic backgrounds who may have access barriers (cultural, language etc) to the mainstream Living Well and other Chronic Disease Management programs.

This project is offered by Calgary Health Region for the two largest ethnic communities in Calgary, the Chinese and Indo-Asian communities.

Living Well with a Chronic Condition

The Chronic Disease Management (CDM) Initiative with the Calgary Health Region began in 2002. The focus of the initiative it to develop a regional strategy for managing chronic diseases. The Region has adopted the Chronic Care Model as its framework. This model has demonstrated that outcomes across a range of chronic diseases can be improved if there is a focus on interdisciplinary teams of providers working together with family physicians, medical specialists and specialty health care teams.

Living Well with a Chronic Condition consists of the following key components:

  • physical activity/exercise
  • health assessment
  • disease specific education
  • chronic disease self-management
  • maintenance
  • liaison with community partners
