Alberta Health Services Community Dental Clinics


To apply to the Alberta Health Services Dental Clinic (for reduced-fee dental care for families in financial need) you will need:

  1. 2009 Income Assessment Form
  2. A form to confirm that you don't have a dental plan.
    If you are attending school click here. If you are working click here.
  3. Evidence of income by providing the clinic with your Notice of Assessment from Revenue Canada. The Notice of Assessment is the letter they send confirming your income tax information. If you do not have your Notice of Assessment, you can phone 1-800-959-8281 and request that an Option C print out be sent to you.

Please note: The dental clinics are backlogged with appointments and accept only a limited number of new patients each month. Once people have started a series of appointments, we work to get them finished before starting another new patient. Please advise staff if you can come to an appointment on short notice.
