Who can Attend?

Healthcare professionals are invited to join the Calgary's Chronic Disease Management Team and attend our program information day to discover how our program started, operates on a daily basis and continues to grow.

Our program information day is designed to provide health care leaders an overview of the Calgary’s Chronic Disease Management Program.

The day will include:

  • Presentations from department managers
  • A folder including all program material, presentation handouts, example algorithms, organizational charts and all program brochures.

    • Whether you are a health care provider or an administrator, you are welcome to attend and have the opportunity to collaborate with our experts and meet leaders from other jurisdictions who may also be attending.
