"Advance care planning" involves talking with your child's doctor about short and long-term care goals when your child has a serious illness. It starts with an open discussion about what to expect from the illness. It can include conversations about treatment options and palliative care (care of patients who have life-threatening illnesses). It can also involve the writing of formal plans and orders.

In the Calgary area, "Goals of Care" provide direction regarding specific health interventions, transfer decisions, locations of care and limitations on interventions for a patient. There are a number of pediatric resources to support your family in the process of determining a Goals of Care Designation for your child if he or she has a serious illness:
 

  • "Hear My Voice" - an activity for families who are coping with a potential life-threatening illness to encourage them to talk about their values and goals to guide decision making during the illness experience.
