The best Asthma Action Plan is personalized for your asthma - that is why you will sometimes hear it referred to as a "self-management plan".  A good plan should include:
	your name
	your contact information/numbers
	you/your child’s goals
	a description of asthma control
	a list of triggers to avoid
	daily medications
	medication adjustments when asthma is changing or getting worse
	the date the plan was completed
