Title:  Evaluation of System-Wide Case Management Project

Background:

·          There are identified information gaps between health service sectors resulting in unmet client needs and fragmentation of service delivery. 

·          Case management is a recommended strategy to address service delivery gaps, but is not consistently available.

·          Alberta Health and Wellness’ goals for case management implementation include: Eliminate gaps between services and promote seamless care; Enhance consistency between regions; Link continuing care services more closely to mental health services and primary health care.

·          The Calgary Health Region has been funded by Alberta Health and Wellness to create, implement and evaluate an 18-month System-wide Case Management Project.

 

Objectives:

·          To evaluate a service model for system-wide case management with 4 pilot populations over a 6 month period and to provide evaluation findings and recommendations to successfully operationalize system-wide case management in the Calgary Health Region

 

Methods:

·          Mixed methods evaluation, including:

·          Interviews and focus groups with project management, steering committee, case managers

·          Surveys and/or interviews with case management clients and informal caregivers (e.g. family members, other unpaid caregivers)

·          Interviews and/or focus groups with clients’ other service providers (e.g. family physicians, occupational therapist)

·          Clinical and administrative data

·          Document review of project documentation

·          4 pilot populations are included: Amyotrophic lateral sclerosis (ALS), Multiple Sclerosis (MS), Cognitive Impairment, Brain Injury/Mental Health

 

Time Frame:

 

The System-wide case management project evaluation occurred from July 2008 to March 2009.
