Objectives:

The intent of this project is to understand the patient experience through the development of comprehensive patient journeys. Domains and sub-domains that reflect the patient experience will be identified to provide a foundation for developing indicators of quality of health care from the patient’s perspective.

 

Background:

 

Patients, particularly those suffering with chronic disease, often see many providers and access services in a variety of different health care setting. A key issue for patients is the continuity of care. Challenges in coordinating services are evident in many areas but is especially significant in managing the boundaries between acute and community-based care. Although continuity of care has received recent significant study, the patient perspectives of outcomes (i.e. quality of care, access, appropriateness, patient safety) is not well documented.

 

Methods:

 

A case study methodology has been used. Four patients diagnosed with Chronic Obstructive Pulmonary Disease (COPD) will be followed for three months post discharge from an acute care medical unit. Data will be collected through patient interviews, patient journals, and health utilization data. A patient journey will be created from the various data components using process mapping techniques. Finally common themes will be extracted from the data to develop domains and sub-domains for potential indicators related to the dimensions of acceptability, accessibility, appropriateness, effectiveness, efficiency, and safety. Also to be explored will be the feasibility of data collection from the various sources and the use of process mapping to recreate and understand a patient’s journey.
