The Personal Directives Act became law on December 1, 1997. The following web sites describe the Act's implications for the individual, for the Agent and for health care and service providers. A link to sample Personal Directives and other resources are provided.
The Personal Directives Act A text (ASCII) version of the Personal Directives Act. Hard copies of the Act are available from any of the CRHA libraries (for CRHA staff), any public library branch and from the Queen's Printer Bookshop.

Choosing Now for the Future: A Guide to Writing Your Personal Directive The electronic version of the pamphlet. Describes what a personal directive is, why one should write one, how to write it, and provides some samples
